About the role

The Boulder Care Advocate (CA) is an important part of a clinical team (physicians, nurse practitioners, and peer recovery specialists) that works together to guide patients with substance use disorder throughout their treatment journey. Backed by exceptional technology, our Care Advocates help participants to navigate traditional systems to overcome access gaps, discontinuity, and stigma. 


Success in this role

  • Partner with patients in their homes, communities, and through the complicated paths of the healthcare system as a trusted navigator for care needs. 
  • Provide telephonic, text-based & video-based (telehealth) engagement and monitoring, assisted by decision-support tools overseen by a prescribing clinician and broader clinical team.
  • Work with patients to achieve their goals, identify new needs, and problem-solve by offering creative solutions to any known or perceived barriers to accessing care and report changes in status to the care team in a timely manner.
  • Coordinate with patients and care teams for planning and delivery by organizing schedules, managing appointments, referrals, and transitions of services and ensuring appropriate follow-up.
  • Conduct self-assessments of strengths and areas for growth with an emphasis on opportunities for efficiency and innovation. 


Essential functions

  • Completes intake visits
  • Completes induction visits
  • Completes and schedules medication adherence monitoring visits 
  • Completes and schedules oral fluid test visits
  • Completes prior authorizations 
  • Communicates with patients via messaging or by phone for any needs regarding care
  • Communication with pharmacies for refills and authorizations



  • Minimum 1-year as a Medical Assistant
  • Must be comfortable with technology and have experience with common business tools (such as Google suite, video meetings, and spreadsheets).


Nice to have

  • Bilingual in English and Spanish preferred (or other language)
  • Minimum of one year experience in navigating the behavioral health system, substance use treatment, patient advocacy, care coordination, crisis hotline, peer coaching, or other relevant experience as a liaison between patients and care teams.
  • Exceptional clinical skills working with a team to achieve outcomes that enrich people's lives by achieving sustained recovery.
  • Lived experience with SUD or AUD recovery (your own or a loved one) is a plus.
  • A passion for serving the needs of people with addiction and mental health challenges, using non-stigmatizing, non-judgmental person-first language.
  • Knowledge of terminology used in healthcare, especially addiction medicine.
  • Team-focused and collaborative. Champions for patients and Care Team members.



  • This position consists of a 9-hour shift (including 1-hour lunch) M-F between the hours of 7am-7pm EST, but it may require some non-standard hours & evenings.
