Boulder Care is looking for a Manager of Care Coordination for a 100% remote, full-time position. Candidates must reside and work in one of the following states to be considered for the role: AL, AK, AZ, CO, FL, GA, ID, MI, NM, NY, NC, OH, OR, PA, TN, TX, and WA

Qualified candidates will have: 

  • 5-7 years of experience in healthcare operations
  • 2–3 years of experience directly managing teams, ex. Care Coordination 
  • A demonstrated ability to improve operational efficiencies
  • Direct experience supporting patients with Substance Use Disorders or working in Medication-Assisted Treatment programs

About the role

The Manager of Care Coordination will lead and oversee the Medical Assistant and Case Management teams, ensuring the efficient and effective delivery of high-quality, patient-centered care. This role involves operationalizing workflows, supporting team growth, and fostering a positive, productive environment. The Manager will play a key role in expanding services and supporting Boulder’s growth, providing oversight of multi-disciplinary teams, implementing complex care strategies, and utilizing a strategic approach to patient care.

 

Essential Functions:

Team Leadership/Development (35%)

  • Lead and manage the Care Advocate and Case Management teams to ensure the delivery of coordinated, comprehensive care services
  • Provide consistent feedback, coaching, and development opportunities for team members
  • Recruit, train, mentor, and retain high-performing team members to support program expansion
  • Align team staffing with organizational and care team priorities
  • Implement regular team check-ins, one-on-ones, and feedback loops to identify and address concerns early
  • Foster a culture where employees feel safe to share ideas, ask questions, and admit mistakes without fear of negative consequences

Operations/Strategic Alignment (50%)

  • Oversee team operations and ensure adherence to organizational protocols and healthcare regulations
  • Collaborate with clinical leadership to align care coordination strategies with broader company goals and initiatives
  • Develop, implement, and optimize workflows that streamline care processes and enhance service quality, support scalability and align with long-term organizational growth
  • Design and implement workflows that Identify potential risks within care coordination workflows and develop strategies to mitigate them

 

Communication/Implementation (15%)

  • Act as a liaison between the care coordination team and other departments to foster effective communication and collaboration
  • Oversee the development and implementation of a resource library to better support patients
  • Create and oversee continuous education programs to keep the care coordination team informed of the latest best practices and healthcare developments
  • Ensure team members have access to training that supports skill enhancement and career growth

 

Qualifications

  • 5-7 years of experience in healthcare operations
  • 2–3 years of experience directly managing teams 
  • Exceptional written and verbal communication
  • Team-focused; effective collaboration. Serves both the team and the customer at a consistently high level
  • Builds high-trust relationships and develops people by listening, coaching, and continuously providing effective feedback

Work environment

  • This is a fully remote role but we are currently only hiring candidates located in the following states: AL, AK, AZ, CO, FL, GA, ID, MI, NM, NY, NC, OH, OR, PA, TN, TX, and WA

    • Applicants must reside and work in one of those states to be considered.
    • Changes to working location require pre-approval from People Operations and are not guaranteed.
  • Office Requirements: Boulder teammates working with sensitive information must have a dedicated, private workspace with a lockable door and high-speed internet to maintain a secure, distraction-free environment, ensuring compliance with HIPAA and confidentiality standards
  • Boulder Care employees are free to use our river-front HQ located in Portland, OR whenever they would like

Expected hours of work

This is a full-time remote position expected to work 40 hours between Monday-Friday during standard business hours.

 Hiring Process

  • First Interview: 30-minute phone 
  • Panel Interview: 60-minute video
  • Final Interview: 30-minute video
  • Anticipated Start Date: January 13th 2025
